It easy to see how a double struck date ( which really is no big deal on most coins ) can wear down on a die then require just one numeral to be re-punched with a number punch can then lead to  a small 8 over a large 8 or in fact any number, this also applies to letters as well. Farthing grades reverse good extremely fine / obverse  extremely fine.
